As Cat Around Town Project is a nonprofit rescue run on volunteers, your assistance in trapping and transporting TNR kitties to a specified location for their surgery is required. Should you need assistance with trapping, please provide reason below.

1. Based on my relationship provided, I have the proper authority to give Cat Around Town Project and it's volunteers permission to Trap, Neuter, and Return cats at the address listed above.

2. Each cat will be vaccinated for Rabies, left ear tipped and spayed or neutered.

4. I release Cat Around Town Project, Board of Directors and Volunteers from any and all liability arising from damages to person(s) or property.
